Audience response questionnaire

Now that I have completed my opening sequence for 'Intruders', I want to hand out a questionnaire to gauge audience response to my opening sequence. This feedback will help me determine whether or not I have achieved my aims. I wanted it to be clear that my opening scene belonged to the horror genre and my prime aim was to unsettle the audience and to create enigma. Below is a questionnaire I will distribute:
