Pre-filming questionnaire

Now that I constructing a rough vision of my opening sequence for 'Intruders', I want to hand out a questionnaire to gauge audience's response on how I should approach filming. This feedback will help me determine whether certain methods are effective and which are not. I wanted it to be clear that my opening scene belonged to the horror genre and my principle aims were to unsettle the audience and to create enigma.
